Catalina Island Health recently launched a new partnership with Welby Health to enhance and manage care for island residents.

Welby Health’s team of experienced healthcare professionals provide personalized support and guidance tailored to an individual patient’s needs. Kind of like concierge care management but it is covered by insurance.

Through this partnership, the team at Welby Health will provide virtual care services to patients with chronic conditions, those transitioning between care settings, and individuals who will benefit from remote monitoring. The goal is to improve overall health and well-being in our community.

How it works

A dedicated nurse care manager will conduct a comprehensive assessment of the individual’s health status, including medical history, current medications, and lifestyle habits. This healthcare professional will work with the individual to identify health goals and develop a personalized care plan.

Services may include:

  • Health education and coaching to help you make informed decisions about your health.
  • Medication management to be sure you understand your medications and are taking them as they were prescribed.
  • Care coordination to connect you with resources and to keep your regular doctor updated in between clinic appointments.
  • Support and encouragement at every step of your healthcare journey.

For patients who require frequent trips to the doctor, this partnership offers the convenient solution of remote patient monitoring. Utilizing user-friendly devices that seamlessly and securely transmit vital health data to your care team, Welby Health can proactively monitor your health, identify potential concerns early on, and adjust your care plan as needed. All from the comfort of your home.

Devices may include blood pressure cuffs, heart rate monitors, pulse oximeters, weight scales, and/or blood glucose monitors.

Remote patient monitoring can be beneficial to patients with various health conditions, such as hypertension (high blood pressure), high cholesterol, diabetes, heart disease, thyroid disorders, obstructive sleep apnea (OSA), kidney disease, asthma, and chronic obstructive pulmonary disease.

As a result, the Welby Health team keeps your regular doctor informed and will help schedule in-person checkups as needed. This combination of remote monitoring and in-person visits ensures you receive comprehensive and personalized care.

In addition, this program is covered as a Med-Cal and Medicare benefit. These services are also covered by most insurance plans and most, or all, costs covered like your doctor visits.

You may have a co-payment or deductible depending on your coverage. If needed, Welby Health can reach out to your insurance carrier to confirm what, if any, out-of-pocket costs you may be responsible for.
